Consultation Information
Your initial appointment will consist of a comprehensive evaluation. It is essential to consider all facts related to your condition.The consultation appointment will give you the opportunity to meet Dr. Snyder and his staff, discuss the surgical procedures, ask questions, and learn what to expect the day of your surgery. Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Referral Slip and X-rays if applicable.
- List of medications you are currently taking
- If you are under the care of a cardiologist please bring the name, address and phone number.
- Any medical and dental insurance information
A parent or legal guardian must accompany all patients under the age of 18 to the consultation visit.
If your dentist or physician has taken X-rays you may request they be sent to our office. If there is not enough time for them to be sent to us via mail, please pick them up and bring them to our office. If your dentist or physician has capability of e-mailing X-rays, they can be e-mailed to us at drsnyder@tcsnyderddsmd.com. If additional X-rays are necessary, they can be taken at our office.
Scheduling Information
We will schedule an appointment for you to be seen as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day. Should you need to change your appointment, please notify our office at least 24 hours in advance.
We believe strongly in the value of your time and will attempt to keep your wait to a minimum. However, emergencies do arise that require our prompt attention. It is reassuring for you to know this in case you are the person involved in an emergency. We appreciate your patience and understanding.

Financial Information
Dr. Snyder’s goal is to provide excellent service at the most reasonable cost. Payment is due at time care is provided. Payment can be made with cash, Visa, or MasterCard.
Your insurance plan is an agreement between you and your insurance company. Our staff will be happy to assist you by submitting your insurance claims. Please bring any information regarding your insurance coverage to your first consultation appointment. At time of service, we collect an ESTIMATED patient portion of your services, based on insurance information received. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We are pleased to offer several health charge plans to help you with financing. If you are interested in applying for a health charge plan, please let us know and we will assist you in the application process.

Patient Protection
We have taken many steps to ensure your safety. Our office exceeds all the standards and requirements for sterilization, disinfections, and contaminated waste removal outlined by the American Dental Association, American Medical Association, Center for Disease Control, and Occupational Safety and Health Administration (OSHA). Your protection is one of our foremost concerns.
